The Hong Kong Jockey Club Community Project Grant: Support and Eductation Programme for Elderly on Use of Medicines (2022-2028) (Ended)
Project Overview
In April 2022, with a generous donation from The Hong Kong Jockey Club Charities Trust, we launched a three‑year service programme providing medication‑related services to community‑dwelling elderly individuals.
Project Objectives
To promote safe and effective medication use, we offer patient‑centered, personalised pharmaceutical services to community‑dwelling seniors in need, addressing medication‑related problems and improving their medication adherence.
Project Vision
Through cross‑disciplinary referrals, we support community health promotion, deliver primary health education, and establish long‑term, sustainable medication management partnerships for the elderly in the community. We also aim to enhance the health self‑management knowledge of both seniors and their caregivers, thereby achieving medical‑social‑pharmaceutical collaboration.
Medication Consultation Service
Service Features
Provided by the pharmacist team:
- One-on-one home-Visit medication consultation
- Explanation of medication uses
- Medication reconciliation
- Personalized Medication Management Service
- Pillbox packaging service (if needed)
Target audience
- Aged 65 or above
- Taking 5 or more medications

Training Course
Course Introduction
3 sessions in total, 2 hours per session
Course Content
- Introduction to commonly used health‑related mobile apps in Hong Kong
- Understanding medication labels
- Distinguishing between adverse drug reactions and side effects
- Learning how to package pillbox
Target audience
- Aged 60 to 69
- Regularly use a smartphone
- Willing to share and help peers
After completing the course
Receive a lucky bag and a certificate of attendance (100% attendance rate)

Thematic Health Talk
Service Features
Delivered by a pharmacist:
- 1‑hour talk and Q&A
- 1‑hour one‑on‑one basic medication consultation
- If there is a high number of medication consultation requests, the pharmacist will later explain the issues over the phone

- Aged 60 to 69
- Have sufficient comprehension ability
- Willing to share and help peers
