The Hong Kong Jockey Club Community Project Grant: Support and Eductation Programme for Elderly on Use of Medicines (2025-2028)

Project Overview

As the population ages, elderly individuals face the challenges of managing multiple chronic diseases and polypharmacy. Some seniors even need to take ten or more types of medication. However, there is currently a lack of professional services in the community to help manage and integrate medications for the elderly, resulting in inadequate support for medication adherence and an increased risk of medication errors.

In response, the Hong Kong Pharmaceutical Care Foundation (HKPCF) applied for and received a grant from The Hong Kong Jockey Club Charities Trust, leading to the launch of a three‑year "Support and Eductation Programme for Elderly on Use of Medicines" in 2022. This programme provides personalised pharmaceutical care to community‑dwelling seniors in need. After receiving the service, participating elderly individuals showed significant improvements in medication adherence and personal health management, clearly demonstrating the programme's effectiveness.

Starting from April 2025, the programme has been extended, and its target population has been expanded to include elderly individuals living alone or in dual‑elderly households who have limited contact with the community, as well as those who are housebound due to illness or disability. The "Support and Eductation Programme for Elderly on Use of Medicines" will continue to provide one‑on‑one support and education through pharmacist home visits, aiming to resolve medication‑related problems among the elderly and improve their health and quality of life.

Project Objectives

To promote safe and effective medication use, and to proactively assist elderly individuals who, for various reasons, are unable to access community resources, by providing them with patient-centered, personalized pharmaceutical services, addressing medication-related problems, and improving their medication adherence.

Project Vision

Through referrals from cross‑disciplinary teams, we aim to support community health promotion, deliver primary health education, establish a long‑term medication support network for elderly individuals in need within the community, enhance the health self‑management knowledge of both the elderly and their caregivers, and achieve medical‑social collaboration.

Target audience

  1. Aged 65 or above
  2. Taking 5 or more medications

How to Apply

  1. Scan the QR code
  2. Fill out the application form
  3. Staff will contact the applicant
  4. Schedule a telephone medication consultation with the pharmacist
  5. Follow up with an home-visit to the elderly (if needed)
